Do you need help or support to remain independent?

Care Coordinators can work with people who have long term conditions or complex care needs. They can find the right level of support to enable you to live as independently as possible, ensuring you receive the right support at a time that is right for you.

Your Care Coordinator can support you with:

  • Coordinating your health care appointments
  • Arranging a care needs assessment
  • Identifying and sourcing equipment
  • Navigation of the health and social care system
  • Recent or new diagnosis of a health condition
  • Unpaid Carers

Minor Surgery by arrangement:

The removal of warts, moles, cysts, ingrowing toenails, verrucae and injection of joints can be undertaken at the surgery, saving you the time and bother of a hospital visit. Please make an appointment to see the doctor if you think you require any of these services.

Cervical Smears: Appointment only

Women aged between 25 & 50 are advised to have a cervical smear every 3 years. Women between 50 and 64 are advised to have a test every 5 years. This is a simple quick test which may detect early changes in the womb. If these are treated straight away cancer may be prevented from ...
