How to Register with Us

Please use the link below to access the New Patient Registration Form online:

Online Service: making it easier for patients to register

We are using a new online service that makes it easy to register with the practice without the need to complete written forms. New patients just need to complete an online form to get started. You do not need proof of address or immigration status, ID or an NHS number. Once you have submitted this form you may need to provide the Practice with suitable proof of ID (Passport, Driving Licence, Bank Statement or Utility bill). If you have repeat medication we may need you to bring a copy of your repeat prescription to enable us to arrange your next prescription more smoothly.

Newly registered patients will be allocated a named GP and offered an appointment for a consultation. You have the right to express a preference to see a particular partner for some or all medical conditions. We will try to ensure that your request is met, but there may be occasions when it is not possible.

The Practice may make use of the new “National Electronic Spine” to obtain your medical records from your previous GP. This system is designed to make your medical history immediately available to your new doctor, improving the service we provide. The Practice is participating in the National Programme for Information Technology, which aims to develop a central electronic database where patient information may be held and transmitted nationally. This is intended to improve patient care and make essential information available wherever the patient might be when treatment is needed.

Patients have the right to opt out of this service – please enquire at Reception for details.

All Registered Patients

Every patient in the Practice has a named GP. This is a doctor who takes responsibility for coordinating your care and won’t necessarily be the person you usually see. If you wish to know who your named GP is, please ask a member of our team who will be happy to help you with this.

Temporary Residents

We are always happy to see any relatives or friends staying with you if they need a Doctor or Nurse. They will need to fill out a short form for the information to be returned to their own Doctor. Please arrive 10 minutes before your appointment to allow time for completion of the necessary forms.

Infectious Disease

If you have a suspected infectious disease, please inform reception if you suspect an infectious disease, as this will enable us to deal with it appropriately during your visit to protect you, other patients and staff.

Consultations for 16 to 75 year olds

If a patient aged between 16 and 75 years has not had a practice consultation within a period of three years, we are happy, on request, to provide a consultation.
