Care Coordinators can work with people who have long term conditions or complex care needs. They can find the right level of support to enable you to live as independently as possible, ensuring you receive the right support at a time that is right for you.
Your Care Coordinator can support you with:
- Coordinating your health care appointments
- Arranging a care needs assessment
- Identifying and sourcing equipment
- Navigation of the health and social care system
- Recent or new diagnosis of a health condition
- Unpaid Carers
If you feel you could benefit from this service, please ask at reception for a referral to your Care Coordinator
